Change of circumstances form

This is the form you need to send off to you LEA (local education authority) if any of their information about you changes. This could be a change of university, change of course, dropping out of university or simply your contact details changing.

Once you've sent this off you don't need to notify any other organisations such as the student loan company as it is done by your LEA.
