Essex accommodation form

The accommodation form was just one page long, though the back was printed with the prices of the various types of accommodation. The first section was just personal details and needed to be completed even if you weren't applying for accommodation.

The second section asked about the types of accommodation you would prefer (smoking or non-smoking, mixed or single-sex) and also which accommodation you would like. You were able to select three choices from the list of six.
